Dear Coach:
We are pleased to be
able to send to you, all of the materials necessary for you to
register for the 2005-6 Indoor Track and Field season. Please
read all of the material carefully since there are important
changes which will affect you and your team.
The enclosed packet
includes the 2005-6 contract which must be filled out in its
entirety and should be returned with all of the supporting
documents outlined below.
NO
contract will be accepted or any team registered for use of the
facility, unless and until the completed contract and the
necessary supporting documents are returned to us.

Contract must be
signed by both the schools Athletic Director and the Head
Coach for all high schools, and by the Club President and
Head Coach for youth clubs.

A team roster
must be typed on the school’s/organizations letter head and
must be signed by the Athletic Director and the Head Coach
for high schools, and the club president and coach for the
Youth Clubs. This roster must also list the names of all
coaches assigned by the school or club. Please note that
any changes to any roster must be submitted to us in writing
from the Athletic Director or Club President only.

Each roster must
be accompanied by a clear photocopy of the athletes’ current
school ID, and a clear photocopy of the coach’s ID.
(Employment, Drivers or Non Drivers ID accepted)

A signed copy of
Armory rules and regulations covering use of the facility is
enclosed and must also be signed by the AD and Coach. Your
signature on this form assumes your acceptance of the rules
and ramifications of any violations.

We must have a
copy of the insurance certificate as per paragraph II of the
contract.

Contract
packages should be returned with all necessary fees.
No school or club
will be considered registered and permitted to use the facility
prior to meeting the above requirements.
